How to Register for 2011-2012 Classes

  1. Download our 2011 Fall Class Schedule (jpg, revised 08-04-11)


    Regarding the Class Schedule:  The class schedule may be amended at any time to better accommodate the dancers and studio.   Classes may be added or they can be changed or cancelled if there are not enough teachers, interest, or students to support it.  The last updated date for the schedule is noted above.

  2. Download and complete the Fall 2011 Registration Form (PDF)
    NOTE:  If you would like to have payments automatically deducted and billed to your credit card, fill out the AutoPay Authorization/Enrollment Form (PDF).
  3. Download the 2011-2012 Rate Sheet (revised Aug 8 2011, jpg). 
  4. Download and sign the Cost/Consent Form (jpg).  
  5. Drop off the following at one of our studio locations (see maps).
    • Completed '2011-2012 Registration Form'
    • Signed 'Cost/Consent Form'
    • The 'AutoPay Authorization/Enrollment Form' if applicable
